Meal Modifications
In some situations, for the health or safety of your student, it may be necessary to request modified meals. This could be due to allergies or other intolerances or medical issues. We want to support you and your student as best we can!
Meal Modification Request Form
If your student has allergies or another need for modified meals, please fill out this form.
Parents should contact the school nurse to explain the modification request and complete the proper documentation (HHSC Dietary Modifications Request form linked above) as necessary. Modifications include substitutes or omissions to the regular meal offerings that meet the student’s individual dietary need.
All requests must begin with the school nurse and are then evaluated by the HHSC Food Service Director. The Food Service Director will discuss available options with parents/student in order to create a specialized diet plan for the student.
Discontinuation of Dietary Modifications Request Form
If your student no longer needs meal modifications, you can fill out this form. After the form has been properly completed, it should be returned to the school nurse.
