About High Ability Forms

Our high ability program has several forms. Depending on your goals, you need to submit specific forms; therefore, we've compiled all high ability forms in one easy location, along with brief directions on how to submit them for consideration.

Request an Appeal

A student, parent, teacher, or administrator can submit an appeal on behalf of a student who does not quality for the high ability program.  The request of appeal must be accompanied by the following documentation and/or samples of student work that demonstrates the student’s competency in the academic areas being evaluated:

  • Documentation and/or samples of student work that demonstrate the student performing at an outstanding level of accomplishment, OR
  • Documentation of performance at or above the 96th percentile on a norm-referenced test of quantitative reasoning ability

HHSC High Ability Contract for Improvement

For a student to continue to be in the high ability program, they must meet certain criteria. For instance, test scores, classroom performance, grades, task commitment, participation, overall academic progress, and overall benefit to the student are all considered.

Should concerns arise, the student may be required to submit a completed a High Ability Contract for Improvement form. The student's parents/guardians will be present and need to sign the document as well.

This document acts as an acknowledgement that the teacher, principal, and parent(s) understand that some changes need to be made in order for the student to continue in the program.

High Ability Exit Request Form

In some cases, it may be that the student does not enjoy the added rigor of high ability classes. Whether it's due to classroom stress or another factor, parents can request the student be withdrawn from the HHSC High Ability Program by filling out this form.
